P1.01-01. Comparison between Protein and Autoantibody Biomarkers for the Early Detection of Lung Cancer

This study compared the effectiveness of protein biomarkers and the autoantibody-based EarlyCDT-Lung test in detecting lung cancer at an early stage. The EarlyCDT-Lung test is a pre-screening test that aims to identify individuals who may benefit from lung cancer screening. The study aimed to evaluate the EarlyCDT-Lung test's ability to predict incident lung cancer in the general population and compare it with a protein-based risk model.<br /><br />The study analyzed data from two European population cohorts, including a development set and a validation set. The characteristics of the study participants, including age, sex, BMI, smoking history, and eligibility for lung cancer screening, were assessed.<br /><br />The results showed that the EarlyCDT-Lung test had a low sensitivity of 14% and a specificity of 86% for incident lung cancer overall. In comparison, the protein-based risk model had a higher sensitivity of 49% at the same specificity level. The protein-based risk model also outperformed the EarlyCDT-Lung test in all evaluated subgroups.<br /><br />Furthermore, the area under the curve (AUC) for the protein-based risk model in the validation sample was 0.75, compared to 0.64 for the PLCOm2012 model. This indicated that the protein-based risk model had better predictive accuracy.<br /><br />In conclusion, the EarlyCDT-Lung test had a low sensitivity in predicting lung cancer when measured in pre-diagnostic blood samples collected up to three years prior to diagnosis. The study found that alternative risk assessment tools, including the protein-based risk model, outperformed the EarlyCDT-Lung test. The study suggests that further evaluation of circulating proteins as biomarkers for lung cancer screening is needed.
