The Spanish Lung Cancer Group conducted a survey to understand the structure and operations of Thoracic Tumor Boards (TTBs) in Spain. The survey was completed by 92 hospitals of various sizes and regions in Spain. The results showed that Medical Oncology is central to cancer care, while resources like Thoracic Surgery, Nuclear Medicine, and Radiotherapy are limited to larger centers. <br /><br />The TTBs in Spain typically hold weekly meetings, with an average of 7 specialists in attendance. The meetings are held in a mixed format, both in person and virtual. The average number of patients discussed per session is 10. Pneumology, Radiology, Pathology, and Medical Oncology are the most commonly represented specialties. <br /><br />The survey also found that 65% of new cases are presented for discussion, and 67% of cases are reviewed before and after surgery. Fast diagnostic pathways are available in 90% of centers. In terms of resources, EBUS, Mediastinoscopy, and NGS are more common in academic centers compared to community centers. <br /><br />The guidelines most frequently referenced in TTB activities include ESMO, SEOM, and NCCN guidelines. The main activities of the TTBs include reaching an agreement on therapeutic strategies, deciding on sample collection techniques, and making staging decisions. However, there is room for improvement in the implementation of timelines and recording outcomes metrics. <br /><br />The survey suggests that specific case managers could be a valuable tool for improvement. It also found that virtual or mixed meetings for TTBs have increased during the COVID-19 pandemic. Overall, the survey highlights the need for equity and improvement in the utilization of new technologies in TTBs in Spain. The study was supported by an unrestricted grant from AstraZeneca Spain.
