EP01.03-002. Implementation of the International Lung Screen Trial (ILST) in Catalonia: A Cost Analysis study

This study analyzes the implementation and cost analysis of the International Lung Screen Trial (ILST) in Catalonia. The ILST protocol involves the use of low-dose computed tomography (LDCT) screening for non-small cell lung cancer (NSCLC) patients. Previous studies have shown that LDCT screening can reduce lung cancer-specific mortality by 20%. <br /><br />The researchers conducted an economic analysis within the Catalan public health system, comparing the costs of the ILST screening program to the usual care for NSCLC patients. The costs included screening, diagnosis, treatment, and follow-up. The distribution of patients in the ILST protocol, based on computed tomography (CAT) results, was considered. The detection rate and the need for a smoking cessation program were based on data from the Germans Trias i Pujol University Hospital. The costs were adjusted based on survival rates according to the AJCC-TNM staging manual.<br /><br />The results showed that the baseline average cost per participant for the 5-year duration of the screening program ranged from 1,054 to 1,832 euros. The average expected cost for a patient in the screening program was 32,431 euros, compared to 91,959 euros in the usual care group. With a 2% detection rate, the expected cost reduction per screening for each participant would be 1,190 euros. The decrease in cost resulting from stage migration offset 88.2% of the screening program costs.<br /><br />In conclusion, the study found that the baseline lung cancer screening costs according to the ILST protocol were relatively low. In the Catalan public health system, the cost reduction from stage migration due to the LDCT screening program compensated for a substantial part of its costs. These findings support the implementation of LDCT screening programs for NSCLC patients in high-risk populations.
