EP01.05-003. Changes of Repeated Lung Cancer Screening Results and Affected Factors; Analysis of Korean National Lung Cancer Screening Program

The Korean National Lung Cancer Screening Program (KNLCS) provides screening tests using low-dose computed tomography (LDCT) every two years to high-risk smokers. A study was conducted to analyze the changes in repeated screening results compared to initial screening and identify factors that affect these changes.<br /><br />The study analyzed data from the National Cancer Center database, which included about 50% of KNLCS participants. In 2019, 50,666 individuals underwent lung cancer screening, and in 2021, 57,578 individuals participated. Out of these, 12,802 subjects were screened in both years. The screening results were categorized as negative (Lung-RADS categories 1-2) or positive (categories 3-4).<br /><br />The results showed that among the subjects who initially had negative screening results, 6.0% changed to positive results in the follow-up screening, while 94.0% did not change. Among those with positive initial results, 81.6% changed to negative results in the follow-up screening.<br /><br />Multiple logistic analysis revealed that age group, number of lung nodules, and the presence of interstitial lung disease were associated with changes in LDCT results from negative to positive. Specifically, the elderly group (aged 65-69 and 70-74), as well as individuals with 1-2 lung nodules or 3 or more nodules, and those with interstitial lung disease, had a higher likelihood of changing from negative to positive results.<br /><br />The study concluded that negative screening results in high-risk smokers can change to positive results in subsequent screenings. Therefore, repeated screening is strongly recommended for this population, particularly for those with relevant factors. Additionally, comparing the readings between initial and repeated screenings can help reduce false positive results.
