EP01.05-005. The Accuracy of Baseline Low-dose CT Lung Cancer Screening: A Systematic Review and Meta-analysis

A systematic review and meta-analysis was conducted to evaluate the accuracy of baseline low-dose computed tomography (LDCT) screening for lung cancer. The study reviewed articles published until April 30, 2021, and extracted data on true positive, false positive, false negative, and true negative results from screening tests. The quality of the literature was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool. Pooled sensitivity and specificity were estimated using a bivariate random effects model, and the area under the curve (AUC) was calculated using HSROC analysis.<br /><br />The analysis included 47 studies with a total of 145,437 individuals. Most of the studies were from Europe and America, with a few from Asia and Oceania. The recruitment period ranged from 1992 to 2018, and the subjects were primarily 40-75 years old.<br /><br />The results showed that the AUC of lung cancer screening by LDCT was 0.97, indicating high accuracy. The overall sensitivity was 0.97, meaning the screening test correctly identified lung cancer in 97% of cases, and the overall specificity was 0.86, indicating that the test correctly ruled out lung cancer in 86% of cases.<br /><br />There was no significant publication bias among the included studies, as indicated by the funnel plot and test results.<br /><br />The study suggests that baseline LDCT screening has high sensitivity and specificity as a screening technique for lung cancer. However, it also recommends long-term follow-up of the entire study population, including those with a negative baseline screen, to enhance the accuracy of the measurements.
