A systematic review and meta-analysis was conducted to compare the efficacy of low-dose computed tomography (LDCT) lung cancer screening in never-smokers versus ever-smokers. The study included 14 studies conducted in Asia, with a total of 141,376 ever-smokers, 109,486 never-smokers, and 1,973 diagnosed lung cancer cases.<br /><br />The results showed that the cumulative incidence of lung cancer among ever-smokers was 1.02%, while it was 0.88% among never-smokers. The highest incidence was observed in female never-smokers (1.55%), followed by male ever-smokers (0.73%), male never-smokers (0.50%), and female ever-smokers (0.05%).<br /><br />The relative risk (RR) of lung cancer diagnosed between ever-smokers and never-smokers was 1.21 for overall, 1.54 for male, and 0.97 for female. Notably, the RR of lung cancer diagnosed among female never-smokers compared to male ever-smokers was 1.28.<br /><br />Proportional meta-analysis also revealed that a significantly higher proportion of lung cancers were diagnosed at the first scan and at early stages (0-1) among never-smokers compared to ever-smokers.<br /><br />Furthermore, the RR for lung cancer mortality and 5-year overall mortality were significantly reduced among never-smokers compared to ever-smokers, with a reduction of 77% and 89% respectively.<br /><br />In conclusion, LDCT lung cancer screening is as efficacious in Asian female never-smokers as it is in Asian male ever-smokers. Lung cancer in never-smokers is a significant global burden, and the study highlights the importance of screening and early detection in this population. The findings also suggest that lung cancer cases diagnosed among never-smokers have significantly reduced mortality compared to those in ever-smokers.
