EP01.07-005. Combined Diffusion-Weighted Imaging and Dynamic Contrast-Enhanced MRI for Diagnosing Indeterminate Pulmonary Nodules

This study aimed to investigate the diagnostic value of combining diffusion-weighted imaging (DWI) with dynamic contrast-enhanced (DCE)-MRI in the diagnosis of indeterminate pulmonary nodules (IPNs) and their subgroups. A total of 123 IPNs were retrospectively identified, and their diagnostic performance was measured using ROC curves. The sensitivity, specificity, positive predictive value, and negative predictive value were calculated. Subgroups of IPNs were also analyzed. The diagnostic performance of DWI and DCE-MRI was compared to positron emission tomography-computed tomography (PET-CT). The results showed that the combination of DWI and DCE-MRI had a powerful performance in diagnosing all IPNs, with a sensitivity of 0.933 and specificity of 0.848. In the subgroups, the sensitivity and specificity were 1.000 and 0.800 in solid IPNs, and 0.948 and 0.875 in subsolid IPNs, respectively. The study also showed that combining DWI and DCE-MRI had an incremental diagnostic value compared to CT alone. Additionally, in a subgroup of 28 patients who underwent DWI, DCE-MRI, and PET-CT, false-diagnoses were more common with PET-CT compared to DWI and DCE-MRI. The authors concluded that the combination of DWI and DCE-MRI was useful in the diagnosis of solid and subsolid IPNs, and could be helpful in the management of pulmonary nodules. The study also found that the apparent diffusion coefficient (ADC) of malignant pulmonary nodules was significantly lower than that of benign nodules, and the distribution of time-intensity curves (TIC) was significantly different between benign and malignant nodules. Overall, this study demonstrates the potential of DWI and DCE-MRI in improving the diagnosis of indeterminate pulmonary nodules.
