EP02.01-010. Preoperative PET-SUVmax and Volume Based PET Metrics of the Tumor Fail to Predict Nodal Upstaging in Early-Stage Lung Cancer

This study aimed to investigate the ability of preoperative PET-SUVmax and volume-based PET metrics to predict nodal upstaging in early-stage lung cancer. The researchers analyzed data from 171 patients with pT1-T2/cN0 lung cancer, specifically adenocarcinoma and squamous cell carcinoma, and examined associations between preoperative PET-SUVmax and volume-based PET parameters with clinicopathological variables and nodal upstaging.<br /><br />The results showed that squamous cell carcinoma and larger tumors were associated with higher preoperative PET-SUVmax values of the primary tumor. Increased preoperative C-reactive protein levels correlated significantly with high preoperative PET-SUVmax values. However, lactate dehydrogenase activity and white blood cell count had no impact on preoperative PET-SUVmax.<br /><br />Furthermore, the study found that squamous cell carcinoma and T2 tumors presented increased preoperative PET-SUVmax values in early-stage lung cancer. However, beyond tumor size and the number of removed lymph nodes, no PET parameters were significantly predictive for nodal upstaging.<br /><br />The overall nodal upstaging rate in the cohort was 14%, with N1 upstaging in 15 patients (8.8%) and N2 upstaging in 9 patients (5.3%). Gender, age, histology subtype, and approach had no impact on nodal upstaging. However, the rate of upstaging was higher in patients with larger tumors, and the number of removed lymph nodes was a predictor of nodal upstaging.<br /><br />In conclusion, this study found that preoperative PET-SUVmax and volume-based PET metrics of the tumor did not predict nodal upstaging in early-stage lung cancer. Tumor size and the number of removed lymph nodes were more significant predictors of nodal upstaging. Further research is needed to identify better predictors for nodal upstaging in these cases.
