EP02.02-006. Differing Doses: The Effects of Radiation Dose Calculation Algorithms on Local Control in Early-Stage Lung Cancer

In this study, the researchers aimed to assess the uncertainties associated with different dose calculation algorithms and analyze their potential impact on the local control of early-stage lung cancer. They analyzed 164 plans for lung stereotactic body radiation therapy (SBRT) for non-small cell lung cancer (NSCLC) that were originally prepared using either the pencil beam convolution (PBC) or the anisotropic analytical algorithm (AAA) calculation algorithms. The plans were then recalculated using the Acuros XB (AXB) algorithm with fixed monitor units (MU), and a subset of plans was also calculated using Monte Carlo for accuracy verification.<br /><br />The results showed that the AXB algorithm had better agreement with Monte Carlo than the AAA algorithm. They also found that higher energies and larger lung volume increased the magnitude of dosimetric difference for the AAA algorithm. Out of the 162 patients, 15 experienced local failure. Univariate analysis revealed that changes in the planning target volume (PTV) and internal target volume (ITV) D1% were associated with an increased risk of failure.<br /><br />The researchers concluded that the choice of dose calculation algorithms in lung SBRT planning could lead to variations in the perception of dose distribution, and these differences may be associated with local control in early-stage NSCLC. They recommend further research on this topic to better understand the impact of dose calculation algorithms on treatment outcomes.<br /><br />This study contributes to the growing body of knowledge on radiation dose calculation algorithms and their effects on treatment outcomes. By identifying potential associations between dose calculation differences and local control, it highlights the need for accurate and standardized dose calculations in lung SBRT planning to ensure optimal treatment outcomes for early-stage NSCLC patients.
