EP02.03-002. Impact of Society and National Guidelines on Patient Selection for Lung Cancer Surgery in the UK from 2008 to 2013

The study analyzed the impact of society and national guidelines on patient selection for lung cancer surgery in the United Kingdom from 2008 to 2013. The aim was to determine if there were any changes in patient selection following the publication of new guidelines in 2010 and 2011. The results showed that there was no overall change in patient selection based on lower levels of lung function after the introduction of the guidelines. The increase in surgical operations was more likely due to greater access to surgery by an increasing number of surgeons rather than the impact of the guidelines. The study also highlighted the variation in the threshold for recommending surgery among clinicians and acceptance among patients.<br /><br />The total number of lung cancer cases diagnosed during the audited period increased, as did the proportion of patients receiving surgery. However, this increase was not associated with lower levels of lung function or an increase in patients with N2 disease. The study suggested that access to surgery and more permissive case selection, rather than guideline recommendations, were responsible for the increasing numbers. The study also found a discrepancy between professional society and national guidelines in recommending surgery for N2 disease, but this disagreement did not have a significant impact on surgical selection.<br /><br />The study acknowledged that the increase in the number of thoracic surgeons in the UK may have influenced surgical selection, as younger surgeons are likely to practice according to their training. The study concluded that the guideline recommendations had little to no impact on patient selection for surgery.<br /><br />Overall, the study showed that despite the introduction of new guidelines, there was no significant change in patient selection for lung cancer surgery in the UK. The increase in surgical operations was not driven by guideline recommendations but by greater access to surgery.
