EP02.03-003. Survival of Patients with Interstitial Lung Disease Who Undergoing Lung Cancer Surgery: A Propensity Score Matching Study

Patients with interstitial lung disease (ILD) who undergo surgery for lung cancer have a poorer long-term prognosis compared to those without ILD, according to a study conducted in South Korea. ILD patients have a higher risk of respiratory complications and increased recurrence rate due to limited resection. The study aimed to compare the clinical outcomes of ILD patients and non-ILD patients after lung cancer surgery by matching variables.<br /><br />The medical records of patients who underwent lung cancer surgery between 2010 and 2020 were reviewed. ILD patients were identified through preoperative CT findings. Propensity score matching was utilized to match ILD patients with non-ILD patients based on age, sex, smoking status, clinical stage, surgical procedure, and pathological diagnosis.<br /><br />The results showed that ILD subtypes were independently associated with prognosis after surgery. The survival of ILD patients after surgery was significantly worse compared to non-ILD patients. Specifically, patients with idiopathic pulmonary fibrosis (IPF) had a higher risk of mortality than the control group, even after adjusting for other factors. Other ILD subtypes did not show a significant difference in survival compared to the control group.<br /><br />The study also compared various variables between the ILD and non-ILD groups before propensity score matching. The ILD group had a higher proportion of male patients, older age, and underwent different surgical procedures and had different pathological diagnoses compared to the control group.<br /><br />In conclusion, ILD patients who undergo surgery for lung cancer have a worse long-term prognosis compared to patients without ILD. Among ILD subtypes, IPF was specifically associated with a poorer prognosis. These findings highlight the importance of careful evaluation and management of ILD patients undergoing lung cancer surgery to improve outcomes.
