EP02.03-010. Intrapulmonary and Hilar Lymph Node Dissection Performed by the Surgical Team: Impact on Pathological Stage of NSCLC

The study compares the lymph node dissection performed by surgeons to that performed by pathologists in patients with non-small cell lung cancer. The objective is to determine the change in lymph node stage from clinical to pathological stage. The study is a prospective cohort clinical study.<br /><br />The surgical group, consisting of 34 patients, had lymph node dissection performed by the surgeon. The control group consisted of 96 patients and historical data from the years 2019 and 2020 when lymph node dissection was performed by the pathologist. The groups were matched based on age, histological type, anesthetic risk classification, clinical staging, invasive staging of the mediastinum, and surgical approach.<br /><br />The primary endpoints of the study were the total number of hilar and intrapulmonary lymph nodes, the number of sampled lymph nodes, the number of affected lymph nodes, and the change in stage from clinical to pathological.<br /><br />After applying the propensity score, 64 patients were selected for the control group. The median age of the patients was 69 years.<br /><br />The study found that the surgeon's lymph node dissection resulted in a greater number of sampled lymph nodes compared to the pathologist's dissection. There was also a higher number of affected lymph nodes in the surgical group. Additionally, the change from clinical to pathological stage (upstage) was higher in the surgical group.<br /><br />In conclusion, the study suggests that lymph node dissection performed by surgeons may result in a more accurate assessment of lymph node stage compared to dissection performed by pathologists. This information could potentially impact treatment decisions and patient outcomes.
