EP02.03-019. T3 Non Small Cell Lung Cancer with Satellite Nodules: A Multicentric Analysis.

This study examines the characteristics and impact on survival of patients with T3 non-small cell lung cancer (NSCLC) and satellite nodules (SN). The researchers collected clinical and pathological data of T3 SN NSCLC patients who underwent surgery between 2005 and 2020. They excluded patients who received systemic treatments in the previous three years, had N3 involvement or stage IV, or had incomplete information. The impact on overall survival (OS) was assessed based on various factors including total tumor size, size of the largest and smallest nodule, and the number of nodules.<br /><br />The study included 56 patients, with 62.5% being male and 69.7% having a smoking history. Preoperative CT scans showed satellite nodules in 50% of patients. Lobectomy was the most common surgical procedure, and an open technique was used in 69.6% of cases. Pathology reports revealed the presence of multiple nodules in 76.8% of patients, with the mean total diameter of the nodules measuring 43.9 mm.<br /><br />The median overall survival in the cohort was 111 months. The total size of the nodules and the size of the largest nodule had a significant impact on OS. Patients with multiple nodules had significantly worse survival compared to those with 2 or 3 nodules. The median disease-free survival was 55 months, and both total size of the nodules and size of the largest nodule significantly impacted DFS. Again, patients with multiple nodules had worse DFS compared to those with fewer nodules.<br /><br />In the subgroup of patients with pN0 status, multiple nodules still confirmed a worse prognosis for both OS and DFS. The study concludes that the total size of T3 NS NSCLC and the presence of multiple nodules are prognostic factors that should be considered in the postoperative management of patients and possibly for upstaging.
