EP02.03-029. Prognostic Factors in Curative Intent Stage I lung Adenocarcinoma: Hospital's Cancer Registry Based Analysis

A retrospective study was conducted to evaluate the overall survival (OS) and prognostic factors in patients with stage I lung adenocarcinoma treated with curative intent radiotherapy and surgery. The study analyzed data from 681 patients diagnosed between January 2000 and December 2015 in São Paulo, Brazil.<br /><br />The study found that patients treated in the Parcial High Complexity Centres in Oncology (PHCCO) network had a lower OS rate compared to patients treated in the High Complexity Centres in Oncology (HCCO) network. Patients who started treatment more than two months after diagnosis had a lower survival rate. <br /><br />Conventional multiple cox regression analysis showed that patients who underwent radiotherapy alone had almost three times the risk of death compared to those who underwent surgery alone. It was also found that patients treated in PHCCO hospitals had a 50% higher risk of death compared to those treated in HCCO hospitals. Older age (60 years and above) and male sex were associated with a higher risk of death.<br /><br />Propensity score analysis revealed that patients treated in PHCCO hospitals and those with a time between diagnosis and treatment of more than two months had a higher risk of death. Patients diagnosed between 2011 and 2015 had a 40% lower risk of death compared to those diagnosed between 2000 and 2005.<br /><br />In conclusion, the study found that the overall survival in patients with stage I lung adenocarcinoma was influenced by the year of diagnosis, the time between diagnosis and treatment start, and the hospital qualification. It was suggested that patients treated in HCCO hospitals who started treatment within one month in the period 2011-2015 had a better prognosis. Further research is needed to validate these findings and explore potential interventions to improve survival outcomes in these patients.
