EP05.01-004. Induction Chemoimmunotherapy before Definitive Chemoradiotherapy for Large-Volume Unresectable Locally Advanced NSCLC

The study investigated the effectiveness of induction immune checkpoint inhibitor (ICI) plus chemotherapy followed by definitive chemoradiotherapy (CRT) in treating patients with large-volume unresectable locally advanced non-small-cell lung cancer (NSCLC). The patients had tumors larger than 5cm in size or metastatic lymph nodes larger than 2cm. The one- and two-year overall survival (OS) and progression-free survival (PFS) rates were assessed. The study also analyzed the dosimetric impact of the treatment on the tumor volume and planning target volume (PTV) using radiotherapy planning.<br /><br />The results showed that out of the 31 patients included in the study, the one-year OS and PFS rates were 96.3% and 81.5%, respectively, and the two-year OS and PFS rates were 80.0% and 46.7%, respectively. The objective response rate of the induction chemoimmunotherapy followed by CRT was 93.5%. The tumors significantly shrank after the first two cycles of treatment.<br /><br />Adverse events were also reported, with grade 2 pneumonitis observed in 22.6% of patients and grade 3 pneumonitis in 3.2% of patients. The dosimetric analysis showed a significant reduction in tumor volume and normal lung protection after the first two cycles of treatment.<br /><br />The study concluded that two cycles of induction chemoimmunotherapy followed by definitive CRT for large-volume unresectable locally advanced NSCLC is feasible and results in significant tumor reduction and normal lung protection. The results warrant further investigations on the combination of CRT with induction and consolidation ICI.<br /><br />In summary, the study findings suggest that induction chemoimmunotherapy followed by definitive CRT shows promise as a treatment approach for patients with large-volume unresectable locally advanced NSCLC, with favorable survival rates and tumor response.
