EP05.01-005. Impact of Antibiotic Use Before Definitive Concurrent Chemoradiation in Patients With Locally Advanced Non Small Cell Lung Cancer

Researchers at the Cancer Center, West China Hospital, SCU conducted a study to investigate the impact of antibiotic treatment on the outcomes of patients with locally advanced non-small cell lung cancer (LA-NSCLC) receiving chemoradiotherapy. The study analyzed the medical records of LA-NSCLC patients who underwent chemoradiotherapy between 2010 and 2017. The researchers examined the use of antibiotics, including the type, duration of treatment, and time between discontinuation of antibiotics and start of chemoradiotherapy. The study found that patients who had received intravenous broad-spectrum antibiotics in the month before chemoradiotherapy had reduced progression-free survival (PFS) and overall survival (OS) compared to those who did not receive antibiotics. Multivariate regression analysis showed that antibiotic treatment was an independent unfavorable prognostic factor for LA-NSCLC patients receiving chemoradiotherapy. The type of antibiotic, length of treatment, and interval between discontinuation and initiation of chemoradiotherapy also influenced prognosis. β-lactamase inhibitors were found to be the most harmful type of antibiotic. The study also found that delaying chemoradiotherapy for 7-16 days after antibiotic discontinuation may reduce the negative effects of antibiotics. The findings suggest that antibiotic treatment for longer than six days, particularly with β-lactamase inhibitors, is associated with a poor prognosis. The study highlights the importance of considering the impact of antibiotic use on treatment outcomes in LA-NSCLC patients undergoing chemoradiotherapy.
