EP05.01-009. A Clinical Outcome Comparison of IMRT and VMAT in Stage II and III NSCLC Irradiated with Radical Intent

This study compared the clinical outcomes of two radiotherapy techniques, Intensity Modulated Radiotherapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT), in patients with stage II and III non-small cell lung cancer (NSCLC). The study included a total of 449 patients treated with IMRT between 2017 and 2019 and 379 patients treated with VMAT between 2019 and 2021.<br /><br />The gender distribution was similar between the two groups, with approximately 48% males and 52% females in both groups. The treatment distribution also did not differ significantly, with 28% of patients receiving radiotherapy alone, 60% receiving concurrent chemoradiotherapy (cCRT), and 12% receiving sequential chemoradiotherapy (seqCRT) in the IMRT group, compared to 33%, 55%, and 13%, respectively, in the VMAT group. The stage distribution was also similar between the two groups, with stage IIIB being the most common stage in both.<br /><br />The World Health Organization (WHO) performance status showed some differences between the two groups, with a higher proportion of patients having a WHO score of 0 and a lower proportion having a score of 1 in the IMRT group compared to the VMAT group.<br /><br />The histology of NSCLC was proven in the majority of patients in both groups, with 54% in the IMRT group and 55% in the VMAT group. Cytology-proven NSCLC was found in 38% and 40% of patients in the IMRT and VMAT groups, respectively.<br /><br />The study showed that there was no significant difference in acute non-hematological toxicity and 90-day mortality between IMRT and VMAT in stage II and III NSCLC treated with radiotherapy alone, cCRT, or seqCRT.
