EP05.01-010. External Validation of Two Prediction Models for Severe Radiation-induced Lymphopenia during Concurrent Chemoradiotherapy for Lung Cancer

This study aimed to validate two prediction models for severe radiation-induced lymphopenia during concurrent chemoradiotherapy (CRT) for stage III non-small cell lung cancer (NSCLC). The two models, developed by the MD Anderson Cancer Center and the Manchester (The Christie) model, were tested on a retrospective single-center cohort of patients who underwent CRT between 2019 and 2021. Lymphopenia, specifically grade 3 or grade 4, was evaluated as the outcome.<br /><br />The study included 100 patients, of which 78% developed grade 3 lymphopenia and 17% developed grade 4 lymphopenia. Weekly decline in lymphocytes was observed throughout the treatment. In terms of model discrimination, both models performed well in predicting grade 3 lymphopenia, with c-statistic values of 0.79 for the MD Anderson model and 0.77 for the Christie model. However, for grade 4 lymphopenia, the MD Anderson model had a higher c-statistic of 0.80, compared to 0.69 for the Christie model. Model calibration, which assesses how well the predicted probabilities align with observed outcomes, showed good calibration for the MD Anderson model in predicting grade 3 lymphopenia and excellent calibration for grade 4 lymphopenia. The Christie model demonstrated moderate calibration for both grade 3 and grade 4 lymphopenia.<br /><br />In conclusion, the MD Anderson model performed better than the Christie model in predicting severe radiation-induced lymphopenia during concurrent CRT for stage III NSCLC. These prediction models may be useful in identifying patients at high risk for lymphopenia and could potentially help improve treatment outcomes and guide clinical decision-making. Further research and validation in larger patient cohorts are warranted.
