Pdf Summary
This study examined the benefit of adaptive radiotherapy (ART) for patients with non-small cell lung cancer (NSCLC), specifically focusing on the different histopathological subtypes. The standard treatment for locally advanced NSCLC is combined chemo-radiotherapy (cCRT), but the patterns of failure differ between adenocarcinomas (AC) and squamous cell carcinomas (SCC). AC is more prone to distant failure, while SCC is more prone to loco-regional failure.

The study included consecutive NSCLC patients treated with curative-intent cCRT before and after the implementation of ART. Patients who were not AC or SCC were excluded. The researchers collected information on patient characteristics, treatment with chemotherapy, administration of immunotherapy for recurrences, and radiotherapy parameters. The patients were divided into two groups based on AC/SCC histology, and overall survival in each group was analyzed using multivariate cox proportional hazard models.

The results showed that after the implementation of ART, there was improved precision of radiotherapy combined with lower normal tissue doses. This led to increased overall survival, but the impact seemed to be dependent on histology. ART primarily led to improved overall survival for patients with SCC.

The study also found that there were no significant differences in baseline patient characteristics between AC and SCC before and after the implementation of ART. However, a higher percentage of patients with SCC in the ART group received immunotherapy as treatment for disease recurrence compared to the pre-ART group.

In conclusion, this study suggests that ART can improve overall survival for patients with NSCLC, particularly those with SCC. Further research is needed to understand the underlying mechanisms and to identify predictive markers for ART benefit in different histological subtypes of NSCLC.

Contact information for the corresponding author, Marie Tvilum, is provided for further inquiries.
