EP05.01-021. Radiation Dose to Cardiac Substructures and the Incidence of Cardiac Events in Patients with Stage III NSCLC Receiving CCRT

A study was conducted to investigate the impact of radiation doses to the heart and cardiac substructures on the incidence of cardiac events in patients with stage III non-small cell lung cancer (NSCLC) who received definitive chemoradiotherapy. The study included 201 patients, of which 43% experienced a cardiac event, with 60% of those being arrhythmias. Patients with a history of cardiovascular disease, higher body mass index, male gender, or hypertension had a significantly greater probability of experiencing a cardiac event after chemoradiotherapy. The study found that certain radiation doses to the left atrium and right ventricle were associated with a significantly increased risk of cardiac events. Specifically, if 1.8% of the left atrium received 60 Gy or any percent of the left atrium received 70 Gy, the risk of a cardiac event was significantly greater. Similarly, if 3% of the right ventricle received 30 Gy, 1.5% received 35 Gy, or 2.25% received either 40 Gy or 45 Gy, the risk of a cardiac event was significantly greater. The study highlights the potential for cardiac toxicities after radiation therapy in lung cancer patients and suggests that radiation dose to specific cardiac substructures may play a significant role in the development of new cardiac events. It emphasizes the need to consider individual risk factors and tailor radiation treatment plans to minimize the risk of cardiac events in these patients.
