Pdf Summary
The 'Italian oBsErvational study on Patient mAnagement strategies in real-world Clinical practIce For patIents with loCally advanced (stage III) NSCLC' (BE-PACIFIC) aimed to describe diagnostic work-up and treatment strategies of stage III Non-Small Cell Lung Cancer (NSCLC) patients in Italy and to evaluate survival outcomes. The study involved 311 patients, with 296 considered eligible and included for analysis.

The median duration of the diagnostic process was 30.4 days, with 48.6% of patients completing the work-up in a single cancer center. A multidisciplinary team (MDT) was involved in 88.7% of patients' treatment plan definition. Among the diagnostic procedures, radiological/imaging examinations for clinical staging and cyto/histopathologic analyses for pathological confirmation were the most performed. PD-L1 level testing was initially done in 196 patients, with 67.9% showing 1% of tumor cells expressing PD-L1. Additional PD-L1 testing was performed in 59 patients during observation.

The study found that 48% of non-resected patients treated with chemoradiation received the programmed death-ligand 1 (PD-L1) inhibitor durvalumab as consolidation immunotherapy. Durvalumab treatment was well-tolerated, and response was observed in 23.9% of patients evaluated for best response. Disease progression or death occurred in 51.7% of patients during the observation period.

Overall, the study highlights the importance of a multidisciplinary approach in the management of stage III NSCLC patients and the use of durvalumab as consolidation immunotherapy following chemoradiation. The findings provide insights into real-world practice and outcomes in stage III NSCLC in Italy.
