EP05.02-015. Neoadjuvant Prehabilitation Therapy for Locally Advanced Non-small-Cell Lung Cancer: Optimizing Outcomes throughout the Trajectory of Care

In this study, the researchers explored the potential benefits of combining prehabilitation with neoadjuvant therapy in lung cancer patients. They retrospectively analyzed 141 patients who underwent neoadjuvant therapy followed by surgery for non-small-cell lung cancer between 2015 and 2021. Out of these patients, 20 were screened for a prehabilitation program. The screening involved assessing their physical performance, nutritional status, and signs of anxiety and depression.<br /><br />Results showed that neoadjuvant prehabilitation therapy was feasible and associated with encouraging outcomes. However, there were logistic challenges in implementing all the measures. Self-reported outcome parameters indicated that 11 patients had elevated anxiety scores, and 6 showed signs of depression. Anxiety levels significantly decreased during the prehabilitation period, but depression levels did not change significantly. The DASI (Duke Activity Status Index) score, which measures functional capacity, markedly improved during the prehabilitation program.<br /><br />The length of stay after surgery was shorter in the prehabilitation cohort compared to the group that did not undergo a preoperative exercise program. The 6-minute walk test, a measure of physical endurance, showed a slight increase after the prehabilitation program but decreased 4 weeks after surgery.<br /><br />These findings suggest that neoadjuvant prehabilitation therapy has potential benefits in improving functional status and reducing postoperative length of stay in lung cancer patients. However, further research is needed, and a prospective multi-center evaluation of this concept is warranted.<br /><br />Overall, this study highlights the importance of multimodal strategies in lung cancer treatment and the potential of incorporating prehabilitation into the care trajectory to optimize patient outcomes.
