EP05.03-001. Impact of Neoadjuvant Chemo/radiotherapy Followed by Surgical Resection on T4N0-1 Non-Small Cell Lung Cancer

The study aimed to assess the impact of neoadjuvant chemotherapy and/or high-dose radiation therapy on the survival of patients with T4N0-1 non-small cell lung cancer (NSCLC). The study included 107 patients who underwent surgical resection for NSCLC between January 2002 and December 2020. Patients with T4N2 disease were excluded from the study.<br /><br />Among the patients, 87.8% received neoadjuvant high-dose radiation therapy and/or chemotherapy before resection, while 12.2% underwent surgical resection without induction chemo/radiotherapy. The patients were followed up for a mean of 66.8 months.<br /><br />The 90-day postoperative mortality rate was 6.1%. The 5-year, 10-year, and median survivals were higher in the neoadjuvant therapy-surgery group compared to the surgery alone group. However, after adjusting for potential confounding variables such as gender, age, tumor size, and nodal involvement, there was no statistically significant difference in survival between the two groups.<br /><br />Furthermore, N1 disease was not found to be an independent prognostic factor for survival. The study concluded that select patients without N2/N3 disease can benefit from surgical resection for T4 NSCLC, and neoadjuvant chemotherapy and/or high-dose radiation therapy may improve survival outcomes.<br /><br />The study highlights the need for further research to identify the best treatment strategy for T4 NSCLC. Despite advancements in thoracic surgery and oncology, the optimal approach remains uncertain. The findings contribute to the understanding of the impact of neoadjuvant therapy in T4N0-1 NSCLC patients and provide valuable insights for clinical decision-making.
