EP05.03-002. Pulmonary Artery Reconstruction for Lung Cancer With N1 Vessel Infiltration: Is It Justified?

This study looked at the use of pulmonary artery (PA) reconstruction in patients with lung cancer. The PA can be infiltrated by the primary lung cancer or by a metastatic lymph node. The researchers compared patients who underwent PA reconstruction for N1 lymph node infiltration versus primary lung cancer infiltration. <br /><br />Between 2018 and 2021, 30 patients with single-N1 NSCLC underwent lobectomy with PA reconstruction. Some patients had received chemotherapy before surgery. The type of PA reconstruction varied, including using patches or conduits. Bronchoplasty was also performed in some cases. <br /><br />The resections were successful in all cases, and the 30-day mortality rate was low. The major complication rate was 10%, but there were no technical complications. The patients had varying pathological stages, and most received adjuvant treatments. <br /><br />The three-year overall survival and disease-free survival rates were 55.2% and 37.9% respectively. The researchers did not find a significant difference in survival between patients with N1 lymph node infiltration versus primary lung cancer infiltration. However, they did identify pathologic N2 occult lymph node metastases and lymphovascular invasion as negative prognostic factors for recurrence. <br /><br />Based on these findings, the researchers concluded that lobectomy with PA reconstruction is a safe and effective treatment for lung cancer, even in cases of N1 infiltration. The study highlights the importance of considering PA reconstruction as a viable option for patients with both primary lung cancer and N1 infiltration.
