EP05.03-003. Superior Sulcus (Pancoast) Tumors: A 11-Year Single-Centre Experience

This study analyzed the outcomes of multimodal treatment for patients with Pancoast tumors (PT) in a single center over an 11-year period. PT refers to tumors located on the lung apex that extend into the structures of the thoracic inlet. The primary objective was to assess the survival of surgical patients with PT.<br /><br />A retrospective cohort study was conducted on patients with superior sulcus non-small cell lung carcinomas who underwent surgery between January 2011 and February 2022. The statistical analysis was performed using IBM SPSS v27.<br /><br />The results showed that the mean overall survival was 115.7 months, with a disease-free survival of 83.9 months. The median age of the patients was 63.5 years, and the majority had a history of smoking with a mean pack-year of 37.5. <br /><br />The overall survival rates at 3 months, 1 year, and 5 years were 88.9%, 63.5%, and 40%, respectively. Common symptoms at presentation included pain (80%) and paresthesia (40%) in the ipsilateral upper limb. <br /><br />All patients received neoadjuvant treatment, and the surgical approach was a Paulson incision. Nine patients underwent lung lobectomy, with en-bloc resection involving the chest wall (80%), brachial plexus (30%), and subclavian artery (20%). In one patient, the surgical procedure was aborted after exploratory VATS. <br /><br />Of the patients who underwent surgery, 8 had R0 surgical histopathology, with stage I, II, III, and IV tumors being represented. The mean hospital stay was 7.6 days, and the mean drainage time was 5.8 days. One patient was excluded due to breast cancer metastasis.<br /><br />The survival curves for PT in this institution were superior to the current literature, suggesting a positive outcome for patients who undergo aggressive surgical treatment. However, the study highlights the importance of selective eligibility criteria for surgical treatment and acknowledges the limited sample size.
