Pdf Summary
The PICTuRE study in Portugal aimed to explore the treatment pathways and outcomes for patients with stage III Non-Small Cell Lung Cancer (NSCLC) who underwent surgery. The study collected data from medical records of Portuguese reference centers and included 284 patients, of which 20.8% underwent surgery.

The surgical patients had a mean age of 65 and were predominantly male (57.6%) and current or former smokers (78.0%). The majority had adenocarcinoma histology (71.2%) and were classified as stage IIIA (79.7%) or stage IIIB (20.3%). The patients had an ECOG performance status of 0-1.

Notably, a significant proportion of patients did not undergo minimally invasive/invasive staging methods to determine the extent of the disease. The median lag times between multidisciplinary team (MDT) decisions and treatment initiation varied among different patient subgroups.

A total of 22 patients received neoadjuvant treatment, with most being stage IIIA. However, the pathologic response to neoadjuvant treatment was poorly reported. The most common surgical procedure performed was lobectomy (76.3%), and the rates of complete (R0) resection were 71.2%, with 13.5% having R1/R2 margins. The operative mortality rate was 3.5%, and morbidity was 21%.

Adjuvant treatment was administered to 84.7% of surgical patients, with chemotherapy being the most common modality. The median overall survival (OS) since diagnosis varied among different patient subgroups, but no statistical differences were observed between most groups.

The study highlights the complex nature of stage III NSCLC and the need for consensus on staging methods. It also suggests the potential for improvement in outcomes for surgical patients. The findings emphasize the importance of an efficient multidisciplinary team approach, with optimal median times for diagnosis, staging, and treatment initiation.

In conclusion, the interim analysis of the PICTuRE study provides insights into the real-world treatment pathways and outcomes for patients with stage III NSCLC who underwent surgery in Portugal. The results underscore the need for well-defined guidelines and a comprehensive approach to improve the care and outcomes for this patient population.
