EP05.03-007. Comparison of Surgical Methods in Patients Receiving Neoadjuvant Treatment; Thoracoscopy vs. thoracotomy

This study compared the surgical methods of thoracoscopy and thoracotomy in patients with advanced stage non-small cell lung cancer who received neoadjuvant treatment. The researchers evaluated the demographic characteristics, post-operative complications, need for additional surgical intervention, amount of drainage, air leak duration, tumor diameter, number of lymph nodes dissected, hospitalization period, and survival of 110 patients who underwent surgery between 2004 and 2020.<br /><br />Among the patients, 53.63% underwent thoracoscopic resection, while 46.36% underwent thoracotomy. The neoadjuvant treatment protocols included chemotherapy (49.1%), chemotherapy and radiotherapy (35.45%), chemotherapy and immunotherapy (8.2%), immunotherapy (5.5%), and chemotherapy, radiotherapy, and immunotherapy (1.8%). The researchers found a statistically significant difference in major complication rates and the need for additional intervention in the postoperative period between the two groups.<br /><br />There was no significant difference in the number of lymph nodes removed or lymph node stations dissected between the two surgical methods. However, in terms of survival, patients who underwent thoracoscopy had a statistically significant advantage compared to those who underwent thoracotomy.<br /><br />In conclusion, the chance of surgery after neoadjuvant therapy should be considered for patients with advanced stage non-small cell lung cancer. The researchers recommend thoracoscopic anatomic resections as the preferred surgical method in cases suitable for surgery, as it provides better outcomes in terms of survival.
