This systematic review and meta-analysis aimed to evaluate the outcomes of extended resection for non-small cell lung cancer (NSCLC) involving the superior vena cava (SVC). Nine observational studies involving 341 patients were included in the analysis. The median or mean age of the patients ranged from 57 to 64 years. Pneumonectomy was performed in 49% of patients, lobectomy in 48%, and sublobar resection in 3%. Among the patients, 74% achieved complete resection (R0). Perioperative therapies included neoadjuvant therapy in 20% of patients and postoperative adjuvant therapy in 37%.<br /><br />The meta-analysis of 315 patients from 7 studies showed a pooled 5-year overall survival (OS) rate of 24.8%. The pooled 3-year OS was 30.8%. Two studies reported 10-year OS rates of 23.1% and 35.4%. Operative mortality rates ranged from 4.7% to 13.3% reported in six studies. The percentage of patients with N2 stage disease was associated with lower 5-year OS. Patients with complete resection were more likely to have a higher 5-year OS, although with borderline significance.<br /><br />Based on the findings, extended resection for NSCLC invading the SVC is considered worthwhile as it is associated with better survival rates and an acceptable morbidity and mortality rate. This review highlights the potential benefits of surgery in carefully selected candidates who may benefit from this treatment approach. It emphasizes the need for further research and larger studies to confirm these findings and further evaluate the outcomes of extended resection for NSCLC involving the SVC.
