Pdf Summary
Researchers conducted a pooled database analysis to determine the key characteristics and clinicopathologic factors that impact survival in patients with thoracic NUT carcinoma (TNUT), a rare and aggressive form of cancer. The study included 141 cases of TNUT, with lung being the most common site of origin. The majority of patients had the BRD4-NUT fusion and presented with metastatic disease. The median overall survival (OS) of the group was 5 months, which was worse than NUT carcinoma in the head and neck region. The median duration of symptoms before diagnosis was 1 month.

Compared to no treatment, various treatment approaches such as surgery, chemotherapy, chemoradiotherapy, and combinations of these treatments were statistically superior and associated with improved median OS. Metastatic disease and BRD3-fusion were found to have a detrimental effect on OS, but the latter did not reach statistical significance. Checkpoint inhibitors and BRD inhibitors were found to positively impact OS, although the latter did not reach significance. Age, sex, and thoracic location did not have a significant impact on OS.

The study highlights that TNUT has a poor prognosis and aggressive multimodality treatment approaches, including checkpoint inhibitors, are associated with better OS. This is the largest cohort of TNUT patients analyzed to date. No conflict of interest was reported by the authors. For contact information, the study can be reached at Philip.Haddad@va.gov.
