Solitary fibrous tumors of the pleura (SFTP) are rare benign tumors that arise from the pleura, accounting for about 5% of all pleural tumors. They are usually asymptomatic and are often discovered incidentally on chest imaging. However, when symptoms do occur, they typically present as a cough, chest pain, or shortness of breath. SFTP can grow to a large size and may demonstrate a pedunculated or sessile growth pattern.<br /><br />Surgical resection is the treatment of choice for SFTP. For pedunculated tumors attached to the visceral pleura, a wedge resection of the lung is sufficient. However, sessile tumors arising on the lung or chest wall usually require a larger lung resection or wide local excision with chest wall resection. Histological examination, including immunohistochemistry, is necessary to confirm the diagnosis of SFTP.<br /><br />In a single-center study of nine patients who underwent surgical resection for SFTP, most cases were asymptomatic, and the tumors were benign in 78% to 88% of patients. Complete en bloc resection was performed in all cases, with varying surgical approaches depending on the tumor type and location. The overall long-term cure rate for all patients was 88% to 92%, although recurrent disease was seen in 8% of cases. Tumor recurrence was more common in malignant SFTP, especially the sessile type, and was associated with a higher risk of death.<br /><br />In conclusion, SFTP is a rare benign tumor of the pleura that usually presents asymptomatically. Surgical resection is the mainstay of treatment and can provide good long-term survival. Recurrence of the tumor is the main risk factor for death, particularly in malignant SFTP, and requires close follow-up and multimodal treatment.
