EP07.01-017. Malignant Fibrous Solitary Tumour of the Pleura Is Not All the Same: Evaluation of Risk Stratification Models in a Large Single Centre Series

Malignant fibrous solitary tumour of the pleura (mSFTP) is a rare disease with unpredictable behavior. Surgery is the standard treatment, but estimating prognosis and planning follow-up is difficult. This study aimed to analyze clinicopathological data of mSFTP to investigate prognostic features and compare three risk stratification models. <br /><br />The study included 34 patients who underwent surgical resection with curative intent. The median follow-up was 111 months. The 5-year overall survival (OS) was 81.2% and disease-free survival (DFS) was 77.4%. 26.5% of patients experienced recurrences. Factors such as calcification, necrosis, and nuclear atypia were independent prognostic factors for worse OS. Radicality of resection, tumor dimension, necrosis, nuclear atypia, and high Ki67 expression were independent prognostic factors for worse DFS.<br /><br />The Tapias score was found to be the best model to predict both OS and DFS in mSFTP. Using this risk stratification model, patients at the highest risk of recurrence could be identified for closer surveillance and longer follow-up. The role of adjuvant treatment for mSFTP has not been established, but further analysis on patients with a high risk of recurrence, stratified according to risk models and biomolecular panels, may inform future post-surgical therapies.<br /><br />In conclusion, this study provides insights into the prognosis of mSFTP and highlights the importance of risk stratification in guiding follow-up and potential treatment decisions. The Tapias score was found to be a valuable tool in predicting outcomes. Further research is needed to determine the role of adjuvant treatment in this rare disease.
