The aim of this study was to compare the outcomes of neoadjuvant chemotherapy (CTx) and neoadjuvant chemo-radiotherapy (CCRT) in the treatment of thymoma and thymic carcinoma. The study included 69 patients who received neoadjuvant therapy between January 2010 and March 2021. The researchers evaluated the resectability, toxicity, radiographic response rate, and downstaging rate following the neoadjuvant treatment.<br /><br />The results showed that there was no significant difference in the resection margin status between the neoadjuvant CTx and CCRT groups. However, the CCRT group had a higher rate of tumor downstaging compared to the CTx group (p<0.001). In the CTx group, 40% of patients showed downstaging, 6% showed upstaging, and 22% showed no response. In the CCRT group, 77% showed downstaging and 23% showed no response.<br /><br />Regarding toxicity, the CCRT group had significantly lower events of neutropenia compared to the CTx group (4.5% vs 29.8%, p=0.040). There were no significant differences in other complications between the two groups.<br /><br />The study concluded that although there was no significant difference in the resection margin status between the neoadjuvant CTx and CCRT groups, the CCRT group showed a higher rate of tumor downstaging and less toxicity than the CTx group. The impact of tumor downstaging on survival needs to be further investigated.<br /><br />In summary, neoadjuvant CCRT resulted in a higher rate of tumor downstaging and less toxicity compared to neoadjuvant CTx in the treatment of thymoma and thymic carcinoma. The study highlights the potential benefits of adding radiotherapy to neoadjuvant chemotherapy in these patients. Further research is needed to explore the impact of tumor downstaging on survival outcomes.
