Thymic epithelial tumors (TETs) are rare tumors of the mediastinum. Currently, platinum-based chemotherapy is the standard of care for advanced thymic carcinomas (TCs), but its efficacy is limited. A phase II trial was conducted to investigate the efficacy and safety of the novel PD-1 inhibitor, toripalimab, in combination with chemotherapy as first-line treatment for advanced TCs. The study is ongoing, but early results show that the combination therapy is efficient and well tolerated. The primary endpoint of the trial is progression-free survival (PFS), and secondary endpoints include objective response rate (ORR), duration of response (DOR), disease control rate (DCR), overall survival (OS), time to response (TTR), and safety. Six patients have been enrolled in the trial so far, with a median age of 46 years. Among the six evaluable patients, 50% achieved a partial response and 50% had stable disease, resulting in an ORR of 50% and a DCR of 100%. The median PFS and OS have not been reached yet. Adverse events were reported in all six patients but were manageable and did not lead to treatment discontinuation or death. Common adverse events included neutropenia, fever, rash, and skin pruritus. The combination therapy of toripalimab and chemotherapy shows promise as a new standard of care for advanced TCs, although more data is needed to confirm these findings.
