EP07.03-007. Clinicopathological Analyses for Predicting Recurrence After Complete Resection of Thymoma

Thymomas are borderline malignant tumors that can recur even after complete resection. This study aimed to identify clinicopathological predictors of recurrence after complete resection of thymoma. The researchers retrospectively analyzed clinicopathological findings and postoperative survival in 126 patients who underwent complete R0 resection for p-stage I-III thymoma at Keio University Hospital between 2003 and 2016. Of these patients, 7 (6%) developed recurrence, with the pleura being the most common site of recurrence. The median time to recurrence was 4.0 years after surgery. <br /><br />Comparisons between the recurrence and non-recurrence groups showed that tumor size, p-TNM stage, and WHO classification were associated with recurrence. All recurrences occurred in tumors that were 3 cm or larger. However, due to the small number of cases with small (3 cm) p-stage II-III tumors, statistical analysis could not be performed. <br /><br />Among the patients with recurrence, one died of thymoma and one died of another disease. Five (71%) of the patients are alive after resection or radiotherapy for recurrent lesions. The median post-recurrence survival was 3.9 years. The study suggests that tumor size may be associated with recurrence, particularly in patients with large thymomas. Close postoperative surveillance and local therapy for recurrence may improve long-term survival in these patients.<br /><br />In conclusion, this study found that the tumor size may be a predictor of recurrence after complete resection of thymoma. Patients with large-sized thymomas may benefit from close follow-up and local treatments for recurrence to achieve long-term survival. Further research with a larger sample size is needed to validate these findings.
