EP08.01-023. Factors Associated with Survival and Refusal of Physician Recommended Immunotherapy in Metastatic Non Small Cell Lung Cancer

This study aimed to identify factors associated with patient refusal of physician-recommended immunotherapy and its impact on survival in patients with metastatic non-small cell lung cancer. The researchers used the National Cancer Database to analyze data from 11,216 patients diagnosed with stage IV NSCLC between 2004 and 2017.<br /><br />The results showed that patients aged 70 or older and those with a higher number of comorbidities were more likely to refuse recommended immunotherapy. On the other hand, patients residing in areas with lower education, Medicare or private insurance, receiving treatment at academic programs, and with adenocarcinoma histology were more likely to accept immunotherapy.<br /><br />In terms of survival characteristics, patients aged 70 or older, with a higher number of comorbidities, and poorly differentiated tumors were associated with poorer survival. Conversely, Asians, receiving treatment at academic centers, and adenocarcinoma histology were associated with better prognosis.<br /><br />The study also found that refusal of recommended immunotherapy was associated with significantly lower median survival compared to patients who accepted treatment.<br /><br />This analysis highlights the importance of understanding patient decision-making and physician-patient communication in establishing trust and adherence to recommended treatment. It also provides insights into the factors that influence patient refusal of immunotherapy and its impact on survival outcomes in metastatic NSCLC.
