Researchers conducted a study to determine if certain subpopulations of peripheral blood mononuclear cells (PBMCs) in patients with advanced non-small-cell lung cancer (NSCLC) are associated with the effectiveness of anti-PD-1 immunotherapy. They analyzed 107 PBMC subpopulations in a group of NSCLC patients before starting immunotherapy, and compared them to a control group of patients receiving non-immunotherapy treatment for advanced malignancies. They found that three subpopulations of cells were associated with significantly different outcomes in the immunotherapy group. These subpopulations were high levels of circulating T helper cells expressing CCR9, high levels of circulating T helper cells expressing CCR10, and high levels of circulating T cytotoxic cells expressing CXCR4. All three of these populations correlated with poorer overall survival in the immunotherapy-treated patients. Importantly, these differences were not observed in the control group. The researchers hypothesized that patients with low levels of these specific lymphocytes, which express chemokine receptors responsible for regulating lymphocyte traffic, have a better outcome because they may experience greater infiltration of immune cells into the tumor, enhancing the effects of anti-PD-1 drugs. The study included 39 patients in the immunotherapy group and 40 patients in the control group, and various baseline characteristics were compared between the two groups. Overall, the findings suggest that high baseline levels of certain T cell subpopulations in the peripheral blood are associated with poorer outcomes in patients with advanced NSCLC who receive immunotherapy. Further research is needed to confirm these findings and better understand the underlying mechanisms.
