EP08.01-101. Factors Predictive of Primary Resistance to Immune Checkpoint Inhibitors in Asian Patients with Advanced NSCLC

This study aimed to determine factors predictive of primary resistance to immune checkpoint inhibitors (ICI) in Asian patients with advanced non-small cell lung cancer (NSCLC). Primary resistance was defined as progression within 6 months of ICI treatment. The study retrospectively analyzed the records of patients who received ICI monotherapy at the National University Cancer Institute Singapore between June 2014 and October 2021.<br /><br />The results showed that elevated Neutrophil Lymphocyte Ratio (NLR) of 3 at the 6th week and female gender were strong predictive factors for developing primary resistance to ICI monotherapy in Asian patients with advanced NSCLC. Female gender had an odds ratio of 5.32, and a higher NLR at the 6th week had an odds ratio of 7.14. Non-smokers and baseline NLR3 did not show significant predictive value.<br /><br />The study excluded patients on dual ICI-combination and ICI-chemotherapy combination, as well as those who discontinued treatment early due to adverse events. The median duration of treatment with ICI was 81 days, and the median duration of follow-up was 295 days.<br /><br />The findings suggest that monitoring NLR levels at the 6th week and considering gender may help identify patients who are likely to have primary resistance to ICI monotherapy. However, larger studies are needed to validate these findings.<br /><br />Overall, this study provides insights into the factors predictive of primary resistance to ICI in Asian patients with advanced NSCLC and highlights the importance of personalized approaches in the treatment of lung cancer.
