EP08.01-107. The Increase of Blood Intratumor Heterogeneity Is Associated with Unfavorable Outcomes of ICIs Plus Chemotherapy in NSCLC

This study aimed to identify potential factors associated with the outcomes of immune checkpoint inhibitors (ICIs) plus chemotherapy for advanced non-small cell lung cancer (NSCLC) patients. Plasma samples were collected from 51 NSCLC patients without certain genetic alterations. The blood intratumor heterogeneity (bITH) score was calculated based on allele frequencies of somatic mutations using a gene panel. The study found that an increase in blood intratumor heterogeneity was associated with unfavorable outcomes of ICIs plus chemotherapy in NSCLC patients. However, neither bITH nor other common molecular biomarkers had an effect on progression-free survival (PFS). The presence of an LRP1B mutation at baseline was significantly associated with favorable outcomes to ICIs plus chemotherapy. Patients with an increase in bITH score of 10% had significantly shorter PFS and a lower durable clinical benefit rate compared to those with a stable or decreased bITH score. A case study also suggested that bITH has the potential to predict disease progression before radiographic assessment. Additionally, the study found an association between LRP1B mutation and other factors such as PD-L1 expression, tumor size, and C-reactive protein level. This research highlights the importance of analyzing blood intratumor heterogeneity as a potential biomarker for predicting treatment outcomes in NSCLC patients receiving ICIs plus chemotherapy.
