EP10.01-016. Cachexia's Impact on Immunotherapy Dose Reduction, Treatment Discontinuation, and Survival: a Systematic Review

The systematic review aimed to assess the impact of cachexia on immunotherapy dose reduction, treatment discontinuation, and survival. Cachexia refers to involuntary weight loss, sarcopenia, and loss of appetite in advanced cancer patients. The review included 25 papers with a total of 3,202 patients, and the majority of the studies used immune checkpoint inhibitors for cancer treatment.<br /><br />The findings showed that cachexia had a negative impact on overall survival in 80% of the analyzed articles, and on progression-free survival in 60% of the articles. However, the impact on time to treatment failure and overall response rate was limited, with only 4% and 32% of the articles respectively showing a negative impact.<br /><br />Data on treatment discontinuation and dose reduction were rarely reported, with only 12% and 4% of the articles respectively showing a negative impact. More data are needed to determine the full impact of cachexia on these outcomes.<br /><br />The review also found that cancer cachexia-related parameters were associated with increased treatment discontinuation, reduced dosing, and worse survival in patients receiving immunotherapy. However, the association between cachexia and reduced survival may be due to the inflammation that drives cachexia inhibiting the ability to mount an effective immune response.<br /><br />In conclusion, the study highlights the negative impact of cachexia on immunotherapy outcomes in cancer patients, particularly in terms of overall survival and progression-free survival. Further research is needed to determine the exact mechanisms behind these associations and to explore the impact of cachexia on treatment discontinuation and dose reduction.
