EP11.03-004. Identification of Filigree Pattern Increases Diagnostic Accuracy of Micropapillary Pattern on Frozen Section for Lung Adenocarcinoma

A study conducted by pathologists at the Shanghai Pulmonary Hospital, Tongji University School of Medicine in China aimed to investigate the diagnostic accuracy of frozen section (FS) evaluation for the presence of micropapillary (MIP) pattern in early-stage lung adenocarcinoma. The presence of MIP has been associated with a poor prognosis in patients undergoing sublobectomy. The study included the identification of a filigree pattern on FS and evaluated its effect on the diagnostic sensitivity and specificity of MIP. The researchers also aimed to verify the prognostic value of the filigree pattern in stage T1 lung adenocarcinoma.<br /><br />A panel of five pathologists evaluated 125 patients with T1 lung adenocarcinoma as a study cohort and 151 patients as a validation cohort. The accuracy of the filigree pattern and classical MIP pattern on FS in predicting the presence or absence of high-risk patterns in permanent sections was investigated.<br /><br />The results showed that the identification of filigree pattern improved the diagnostic sensitivity of MIP pattern on FS. The study found that FS was feasible for detecting both filigree and classical MIP patterns in stage T1 lung adenocarcinoma.<br /><br />In conclusion, the study suggests that the inclusion of the filigree pattern on FS can improve the diagnostic accuracy of MIP pattern in early-stage lung adenocarcinoma. This could have implications for the prognosis and treatment of patients with this type of cancer. Further research is needed to validate these findings and understand the underlying mechanisms of the filigree pattern.
