EP13.01-003. Diagnostic Yield of Radial Probe Endobronchial Ultrasonography without Fluoroscopic guidance in Peripheral Pulmonary Lesions

This study aimed to determine the diagnostic yield of radial probe endobronchial ultrasound (R-EBUS) without fluoroscopy guidance in diagnosing peripheral pulmonary lesions (PPLs). A systematic review and meta-analysis were conducted, including 31 studies involving 6,491 participants.<br /><br />The overall pooled diagnostic yield of R-EBUS guided transbronchial biopsy (TBB) without fluoroscopy was found to be 0.70 (95% confidence interval, 0.67-0.74). However, there was substantial heterogeneity among the included studies, so the results should be interpreted with caution.<br /><br />Several factors were found to have a statistically significant impact on the diagnostic yield of R-EBUS without fluoroscopy guidance. PPLs with an air bronchus sign on chest CT had a higher diagnostic yield compared to those without (0.81 vs. 0.46, p<0.001). Lesions located in the upper lobe had a lower diagnostic yield compared to those in the middle or lower lobes (0.71 vs. 0.76, p=0.046). Larger size lesions (cutoffs of 3 cm and 2 cm) were associated with a higher diagnostic yield compared to smaller size lesions (p<0.001). Placement of the probe within the lesions yielded higher diagnostic accuracy compared to placing it adjacent to the lesions (0.81 vs. 0.47, p<0.001).<br /><br />The incidence of pneumothorax and significant bleeding ranged from 0.60% to 5.12% and from 0% to 8.12%, respectively, among the included studies.<br /><br />In conclusion, R-EBUS guided TBB without fluoroscopy appears to be a useful tool in the diagnosis of PPLs. However, the diagnostic yield is influenced by factors such as the presence of air bronchus sign, lesion location, size, and probe placement. The study highlights the need for cautious interpretation due to heterogeneity among studies.
