EP14.01-012. A Case Report of Lung Large Cell Neuroendocrine Carcinoma with Carcinoid Morphology

This case report discusses the diagnosis and treatment of a rare form of lung cancer known as lung large cell neuroendocrine carcinoma with carcinoid morphology. The patient, a 69-year-old male smoker, presented with a persistent cough and a history of hypertension. Upon imaging, a central tumor mass was identified in the left lower lobe of the lung. The patient underwent surgery, specifically left lobectomy and mediastinal lymph node removal, with no complications. The tumor, measuring 2.6cm in diameter, exhibited a solid growth pattern with round-to-ovoid tumor cells and scant cytoplasm. Immunohistochemical analysis confirmed the presence of CD56, chromogranin, synaptophysin, and TTF-1 proteins. Necrotic areas were observed, but there was no lymphovascular or pleural invasion identified. The mitotic count was 11/2mm2, and the Ki-67 index was 40% in hot spots. The tumor was classified as large cell neuroendocrine lung cancer with carcinoid morphology stage IA(pT1c pN0) according to the World Health Organization (WHO) classification. The patient did not receive any adjuvant therapy, and 25 months after surgery, no relapse of lung cancer has been observed. The report concludes that lung large cell neuroendocrine carcinoma with carcinoid features is a rare entity, and there is still a lack of clinical, pathological, and genetic data to guide the treatment of these patients. Several references are provided for further reading on the topic, including studies on the morphologic and molecular classification of lung neuroendocrine neoplasms and the role of the Ki-67 antigen in the WHO classification of lung neuroendocrine tumors.
