Pdf Summary
A 68-year-old male patient with large cell neuroendocrine carcinoma was undergoing treatment with immunotherapy when he developed immune thrombocytopenia, a rare and potentially life-threatening adverse event. The patient's complete blood count revealed low platelet levels, confirming the diagnosis. Further investigations ruled out other potential causes and determined that the thrombocytopenia was related to the immunotherapy. The patient was initially treated with corticosteroids but showed no improvement, so the corticosteroid dose was increased and intravenous immune globulin was administered. After ten days, the patient's platelet count stabilized, and he was discharged from the hospital. It was decided to permanently discontinue the immunotherapy. The patient is currently on follow-up, with tapering of corticosteroids, and his platelet count remains within the normal range. The most recent restaging showed a partial response to treatment. Prior to developing immune thrombocytopenia, the patient was enrolled in a clinical trial and was receiving concurrent chemoradiation and immunotherapy. He showed a partial response to the treatment and continued with maintenance therapy. This case highlights the importance of recognizing and managing immune-related adverse events associated with immune checkpoint inhibitors, as they can cause significant complications. Immune thrombocytopenia should be considered as a potential diagnosis in patients undergoing immunotherapy who present with low platelet levels. Prompt treatment with corticosteroids and immune globulin can lead to improvement in platelet counts and resolution of symptoms. Discontinuation of immunotherapy may be necessary to prevent further adverse events.
