EP14.01-024. Transformation of SCLC to Lung Squamous Cell Carcinoma after First-Line Chemotherapy and Response to Sintilimab: A Case Report

This document is a case report of a patient who experienced a histological transformation from small cell lung carcinoma (SCLC) to lung squamous cell carcinoma (LUSC) after receiving first-line chemotherapy. The patient initially presented with epigastric distension discomfort and was diagnosed with stage IV SCLC of the right lung. They received cisplatin combined with etoposide, but after four cycles of treatment, the disease progressed with skull metastases. The patient underwent radiotherapy and the disease stabilized.<br /><br />Three years later, the patient developed a new lesion in the left lung, which was identified as LUSC through repeat biopsy. The patient was then treated with sintilimab, an immunotherapy drug, based on higher tumor mutation burden and the absence of a standard treatment regimen. During the immunotherapy, the tumor lesion in the right lung significantly decreased in size, while the lesion in the left lung remained stable. The progression-free survival of the patient was almost 8 months.<br /><br />This case report is significant because it is the first documented report of histological transformation from SCLC to LUSC. It highlights the importance of repeat biopsy in managing disease progression and emphasizes the need for further research and understanding of such transformations. The response to sintilimab in this case demonstrates the potential efficacy of immunotherapy in treating LUSC.
