EP14.05-020. Population-based Outcomes for Patients with Extensive-Stage Small-cell Lung Cancer from the Canadian SCLC Database (CASCADE)

The Canadian Small Cell Lung Cancer Database (CASCADE) is a multi-institutional real-world evidence (RWE) database of patients with small cell lung cancer (SCLC) from 8 academic institutions across Canada. The preliminary data from three sites (BC Cancer, Princess Margaret Cancer Centre, and the Ottawa Hospital Cancer Centre) shows that population-based survival for patients with extensive-stage (ES) SCLC is poor. Many patients do not receive any systemic therapy, leading to extremely poor survival outcomes. Only a minority of patients are treated with second-line systemic therapy. The uptake of consolidative radiotherapy and prophylactic cranial irradiation (PCI) is low in this real-world population.<br /><br />The study included 1,225 patients with ES-SCLC. The median age at diagnosis was 68 and the majority of patients were diagnosed between 2011 and 2015. 48% of the patients were male and 52% were female. 62% of the patients had a smoking history, with 62% being current smokers, 38% former smokers, and 1% never smokers. The vast majority of patients (95%) were diagnosed with AJCC stage IV disease. The ECOG performance status of the patients was 0-1 in 64% of cases and 2 in 36% of cases.<br /><br />The median overall survival for patients with ES-SCLC was 6.5 months, with a 1-year survival rate of 24% and a 2-year survival rate of 7%. The use of thoracic radiotherapy and systemic therapy varied among patients, with 90% receiving thoracic radiotherapy and 10% receiving systemic therapy. The median overall survival for patients who received thoracic radiotherapy was 8.5 months, compared to 1.0 month for those who did not receive thoracic radiotherapy. Similarly, the median overall survival for patients who received systemic therapy was 19.0 months, compared to 12.0 months for those who did not receive systemic therapy.<br /><br />In conclusion, the population-based outcomes for patients with extensive-stage SCLC in the Canadian Small Cell Lung Cancer Database (CASCADE) are poor. Many patients do not receive any systemic therapy and have extremely poor survival. The use of consolidative radiotherapy and PCI is low in this real-world population. The study will continue to report on this population as data from five additional CASCADE sites becomes available.
