EP16.01-017. T-cell Repertoire Heterogeneity and Homogeneity in Synonymous Multiple Primary Lung Cancers

Multiple primary lung cancers (MPLCs) are becoming more common with the increasing use of lung cancer screening. However, MPLCs have a highly heterogeneous immune profile, which limits the effectiveness of immune checkpoint inhibitors (ICIs). Understanding the immune contextures of MPLCs is crucial for developing effective treatment strategies. <br /><br />In this study, researchers analyzed the genomic and immune characteristics of tumor samples from 27 patients with synonymous MPLCs and compared them to samples from patients with a single pulmonary nodule (SN). They used next-generation sequencing, immunohistochemistry staining, and T-cell receptor (TCR) sequencing to assess the T-cell repertoire in these samples. <br /><br />The results showed that while there were no differences in PD-L1 expression and tumor mutation burden between MPLCs and SNs, there were significant differences in TCR repertoire. MPLCs had higher TCR diversity and comparable TCR expansion compared to SNs. T-cell diversity and reactivity in MPLC lesions were found to correlate with clinical and molecular features, indicating a distinct tissue microenvironment in MPLCs. <br /><br />Although there was heterogeneity in the TCR repertoire among different lesions within the same patient, the fittest clones, or the most dominant T cells, were consistently present and expanded across lesions. This suggests convergent molecular events in MPLC tumors and may help in identifying biomarkers for immunotherapy in MPLCs. <br /><br />The study highlights the importance of considering the immune contexture of MPLCs in treatment strategies. By understanding the heterogeneity and homogeneity of the T-cell repertoire, researchers can potentially develop more effective immunotherapies for MPLCs. This knowledge may also aid in the identification of biomarkers for patient stratification and personalized treatment approaches.
