EP16.01-029. Inter-tumor Heterogeneity of PD-L1 Expression in Non-small Cell Lung Cancer

This study aimed to investigate the concordance rate of programmed cell death ligand 1 (PD-L1) expression between primary tumors and metastatic lymph nodes in non-small cell lung cancer (NSCLC). The study used the 22C3 PD-L1 immunohistochemistry assay to evaluate PD-L1 expression in tumor tissues from primary tumors and paired metastatic lymph nodes. The samples were obtained from 76 patients who underwent surgery between August 2016 and April 2021. The results showed that the concordance rate of PD-L1 expression between primary tumors and metastatic lymph nodes was 43.4%, indicating inter-tumor heterogeneity of PD-L1 expression. The distribution of PD-L1 expression levels was also analyzed, with 28.9% of primary tumors showing high expression, 36.8% showing low expression, and 34.2% showing no expression. In metastatic lymph nodes, 15.8% showed high expression, 36.8% showed low expression, and 47.4% showed no expression. The study concluded that PD-L1 expression is heterogeneous among different tumors, and further investigation is needed to determine a better biomarker for immunotherapy. The findings suggest that the choice of tissue for testing PD-L1 expression in NSCLC should be carefully considered. The study was supported by grants from the Saitama Cardiovascular and Respiratory Center. References to previous studies and guidelines on PD-L1 expression testing in lung cancer were also provided.
