EP16.02-008. Cerebrospinal Fluid as a Liquid Biopsy for Molecular Characterization of Brain Metastases in Patients With Non-small Cell Lung Cancer

Researchers have investigated the use of cerebrospinal fluid (CSF) as a liquid biopsy for molecular characterization of brain metastases (BM) in non-small cell lung cancer (NSCLC) patients. The study included 5 NSCLC patients with tumor material from primary tumor, BM, plasma, and CSF. The researchers performed a combined analysis of circulating tumor DNA (ctDNA) and exosomal RNA in CSF and compared it to the other compartments. They used Illumina Trusight tumor170 kit for library construction and sequencing. The results showed a high degree of overlap in variants between the primary lung tumor and brain metastasis, while there was relatively less variant overlap between plasma and CSF. Specific mutations were observed in the CNS compartments, which could serve as potential targets for tailored therapies. The researchers concluded that the combined DNA and RNA analysis of CSF can be a surrogate for BM biopsy and guide treatment decisions for NSCLC patients with BM. Further validation in larger clinical trials is needed.<br /><br />The study also provided an overview of the workflow, from nucleic acids isolation to library construction and sequencing. The quantity and enrichment of mapped reads in different biological compartments were analyzed, and the sequence coverage in those compartments was evaluated. The overlap of variants in different compartments was determined, and phylogenetic trees were generated to show common variants and variants specific to lung and BM compartments.<br /><br />Overall, this study highlights the potential of using CSF as a liquid biopsy for molecular characterization of BM in NSCLC patients. The findings have implications for personalized treatment options and may contribute to improved outcomes for these patients.
