A study conducted across nine cancer centers aimed to assess patient-reported experiences in order to inform the implementation of the 4R Oncology model of patient self-management for non-small cell lung cancer (NSCLC) patients. The 4R Oncology model has previously been successful in improving access to guideline-based care for breast cancer patients. However, its effectiveness in NSCLC patients has not yet been studied. <br /><br />The study surveyed 200 NSCLC patients across four community centers, three academic centers, one VA center, and one integrated center between January 2021 and March 2023, before the implementation of the 4R model. <br /><br />The results showed that 42% of patients reported being at stage IV, 37% reported stages I-III, and 21% were unsure of their stage. When asked about their care goals, 43% reported aiming for a cure, 38% reported non-curative goals, and 20% were unsure. It was found that 18% of patients had care goals that were inconsistent with their stage. <br /><br />The study also revealed that early-stage patients were referred to smoking-cessation, primary/chronic care, and nutrition services at a higher rate than stage IV patients. On the other hand, stage IV patients were referred at a higher rate for emotional and practical support. <br /><br />The findings of the study highlight the need for improvements in guideline-based care for NSCLC patients. It is concerning that one-fifth of patients were unaware of their stage or care goals. Additionally, fewer than half of the patients were referred to practical, nutrition, or emotional support services. To address these issues, the implementation of 4R care sequences is being pursued. Figure-1 depicts the proposed care sequences.
