This study aimed to investigate the effects of preoperative hematocrit levels on perioperative outcomes and survival following robotic-assisted video thoracoscopic pulmonary lobectomy. The researchers analyzed data from 726 patients who underwent this procedure over a period of 11.8 years. The patients were divided into two groups: anemic (with hematocrit levels below 36% for women and 41% for men) and non-anemic (with hematocrit levels above these thresholds). <br /><br />The results showed that the anemic group had a higher proportion of male patients, while the non-anemic group had a higher proportion of female patients. Anemic patients also had a greater incidence of preoperative weight loss compared to non-anemic patients. During the surgery, anemic patients experienced higher estimated blood loss and received a greater amount of intravenous fluids. They also had longer skin-to-skin operative times, longer chest tube duration, and longer hospital stays compared to non-anemic patients. The anemic group had a higher rate of intraoperative complications, with a conversion rate to thoracotomy almost three times that of the non-anemic group. <br /><br />Postoperatively, anemic patients had higher incidences of aspiration and pneumothorax after chest tube removal. The overall median survival for anemic patients was significantly lower than that of non-anemic patients. In conclusion, anemic patients undergoing robotic-assisted pulmonary lobectomy experienced longer operative times, increased blood loss, higher rates of complications, and poorer survival outcomes compared to non-anemic patients. These findings suggest that preoperative hematocrit levels can impact perioperative outcomes and survival in this surgical procedure.
